ACKNOWLEDGMENT OF RISKS — Participation in training sessions, fitness assessments and related activities carries inherent risks, including physical injury (sprains, strains, fractures), cardiovascular complications, property damage, and other risks associated with exercise and physical exertion. I voluntarily assume all such risks.
HEALTH AND MEDICAL CONDITIONS — I confirm I have disclosed all known medical conditions, injuries or limitations to ODH Fitness LLC & OPDH Player Project LLC , and I agree to notify my trainer immediately if any condition changes or new concerns arise.
WAIVER AND RELEASE — In consideration of being allowed to participate, I release, waive and discharge ODH Fitness LLC & ODH Player Project LLC , Omar Holness and all affiliates from any and all liability, claims, demands or causes of action arising out of or relating to my participation, including any injury, illness or loss of property. This release extends to claims caused by negligence, active or passive.
RESPONSIBILITY AND CONDUCT — I agree to follow all instructions carefully, to inform my trainer of any pain, discomfort or concerns during training, and to conduct myself safely and respectfully. Any exercise may be stopped at the trainer's discretion if deemed unsafe.
PHOTOGRAPHY AND MEDIA — ODH Fitness & ODH Player Project may request to use photos or video of training sessions for marketing or social media. Consent is optional and may be withdrawn at any time.
PAYMENT AND SCHEDULING — I acknowledge that sessions are prepaid according to agreed terms, and I understand the rescheduling and cancellation policies outlined by ODH Fitness & ODH Player Project LLC.
ACKNOWLEDGMENT OF UNDERSTANDING — I have read this waiver carefully, fully understand its contents and agree to all terms. Signing is a condition of participation. If the participant is under 18, a parent or guardian must complete and sign this form.
